Diacylglycerol (DAG): Efficacy and Mechanism of Action of an Anti-Obesity Agent. Obesity Research 13:1864-1876 (2005) is a review of this agent that appears to be effective, inexpensive and non-prescription.
DAG oil is believed to increase B-oxidation, enhance weight loss, suppress fat accumulation and to lower some blood fats after meals. Research is spotty on weight loss success because weight loss is a multifactorial process. It seems to work within the framework of your specific problems.
Its main action makes you full quicker in addition to the above good effects. Safety studies show no apparent risks. It is being used as an additive in products for weight loss. Look for it on the label.
This is all part of the trend to find agents that can be incorporated in food and will work on the brain to suppress appetite as well as to other good things for your metabolism.

Metabolic Syndrome and Fat
The world is looking at the “Metabolic Syndrome” with news eyes. Insulin resistance defined it before as one of the essential factors without which you did not have the “Metabolic Syndrome”. But the Japanese study published in Diabetes Care 29: 145-147, 2006 has opened a new question.
It appears that if the patients have insulin resistance they will have increased heart disease. But if you only use waist circumference that is supposed to signal increased brown fat and insulin resistance, you do not necessarily have increased heart disease.
It may be that you just seem to have increased fat, not brown, and that’s all.
Does this signal the “Metabolic Syndrome subgroup B”?

There was a recent publication about people with mild diabetes that showed results of a recent two-year study assessing the impact of conventional weight loss counseling versus intensive combination medical and behavioral therapy (using sibutramine or Meridia) on body weight.
After one year all subjects were converted to the intensive combination therapy approach. Primary outcome measures include weight change and effect on sugar control as measured by HgA1c. This is one of few studies to report long-term (> 1 year) follow up of weight loss in individuals with type 2 diabetes. Conventional therapy subjects did not achieve significant weight loss at one year. In contrast, the combination therapy group achieved a mean weight loss of 8 kg.
Not surprisingly, in the second year of study, the combination therapy subjects experienced modest weight regain with weight loss at the end of the second year averaging only 5 kg.
Using Byetta in these patients we have had a much greater weight loss. With the mechanism of weight loss we believe it will not come back. We will need about two years to see it that is true.

In fat cells, activation of CB1 promotes more formation of fat and inhibits production of the beneficial hormone adiponectin that removes fat.
Rimonabant represents the first specific CB1 blocker to enter clinical development. Other studies have also shown a favorable affect on smoking cessation in subjects treated with rimonabant. N Engl J Med 2005 11; 353(20):2121-34.

Do Dairy Products Promote Weight Loss?
We have heard since 2000 that dairy products promote weight loss. Now a very large study in men published in the March issue of the American Journal of Clinical Nutrition says it is not so.
Another researcher at Creighton University says there is a Calcium effect but it is very small.
We have had one person lose about 30 lbs with just Vitamin D I'm not sure of what to make of that.

Who can benefit from Byetta? This is a new self injected hormone that has had success in treatment of diabetes and created significant weight loss.

Clinical Diabetes Vol 24, #1, 2006, pg18-24 had a rather extensive coverage on the metabolic effects of the antipsychotic Medications. Some of them essentially make you have diabetes. The first generation of these agents have most of these effects but it was hoped that the second generation would not have such problems. They do.
The first generation drugs are Chlorpromazine, Perphenazine, Fluphenazine, Thiothixene, and Haloperidol. They all cause weight gain and if you’re genetically inclined, diabetes 2.
The second generation drugs used often today have increased risk of weight gain, diabetes and elevation of triglycerides.
These medications have been noted to be associated with an increased incidence of stroke. The names of these drugs are Risperidone, Olanzapine, and Aripiprazole. At worst the risk only increases to a little over 1% but it is nice to know.
The increased incidence of cardiovascular disease with diabetes is thought to be due to small vessel disease, the hallmark of diabetes. You can be tested for small vessel disease with many different office-based machines and if at risk dramatically decrease your risk of stroke with l-arginine 5 gm daily.
Those who take these drugs have to. Thank God we have them. You can mitigate the risk. It’s your time.

The epidemic of those with increased weight has been recognized the world over. More than 1 billion people world wide are overweight and 300 million obese. The developed countries seem to be the hardest hit.
It is worth while looking at a definition of obesity. It is excessive fat from a chronic imbalance of energy. In this there is all the usual stuff about diet and exercise but this balance omits reference to cause. In the population I deal with, there seem to be two big problems.
First are those who just over eat. They first are blamed for all that it is bad about the “obesity epidemic” with the assumption that it is only a “balance” problem easily solved with the increased exercise and decreased eating that is always suggested. It is not that simple.
It is often said that there is “a lack of safe Pharmaceutical agents” and therefore more research on new drugs is needed. That will be the subject of the rest of this White Paper, but we do have a number of very effective medications that are out of style for most doctors.
Second, we have the internal problem that there has been a trigger of some sort that more many people that leads to certain disorders. Xenical Stopping Cancer
Last week I spoke of Xenical as being a very good agent to stop ovarian cancer. This week I want to talk about Xenical a very good agent to support the progression of breast cancer. Xenical is a weight loss medication that is turning out to be worth so much more. Annals of Oncology, 2005 Aug; 16(8):1253-67
Cancer seems to go from genes that are doing the wrong thing and then suddenly become something that is often very aggressive from the genetic standpoint. Xenical causes a dramatic down regulation of certain genes that promote cancer. This is a 90 percent reduction in the gene expression.
This appears to attack certain genes in certain cancers but it seems to be greater than 90 percent effective in stopping these cancers. Wow.
You may want to be slim but you really do not want breast cancer; or ovarian cancer. It's your time.
